Uniform Final Reports

Section 602 of the Bankruptcy Abuse Prevention and Consumer Protection Act of 2005, codified at 28 U.S.C. § 589b, required the Attorney General to issue uniform forms for final reports by trustees under chapters 7, 12, and 13 of the Bankruptcy Code (uniform forms). The uniform forms were developed through the Federal Rulemaking process, and the initial forms took effect on April 1, 2009. Based on comments from users, modifications have been made to the forms.The modified uniform forms take effect on October 1, 2010.

Trustee Final Report Generation System (TFRGS)

To assist those chapter 7, 12, and 13 trustees who do not have access to software that will produce the required data-enabled final report forms, the United States Trustee Program (USTP) developed the Trustee Final Report Generation System (TFRGS).  TFRGS is an application that contains fillable PDF versions of each of the seven applicable uniform final report forms. Using Adobe Reader, trustees can download these forms from the USTP Web site, enter the required data, save the files to their workstations, and then upload them to the bankruptcy courts as data-enabled PDF files.

Privacy Act Statement

28 U.S.C. § 589b (a)(3)(A) authorizes the collection of individual debtor and other information that you are required to provide pursuant to 11 U.S.C. § 704. The United States Trustee will use the information collected to evaluate your administration of cases and debtors’ progress through the bankruptcy system in order to satisfy its statutory oversight duties. The United States Trustee will not share the information with any other entity unless authorized under the Privacy Act, 5 U.S.C. § 552a, et seq., and as may be permitted by the routine uses enunciated in the United States Trustee Program’s system of records notice, UST-002, “Bankruptcy Trustee Oversight Records.” See 71 Fed. Reg. 59,818, 59,822-59,823 (Oct. 11, 2006). Additional disclosure of the information may be to district and regional offices of each United States Trustee. Failure to provide the required information may result in suspension or termination of your appointment to the panel pursuant to 28 C.F.R. § 58.6.
